To provide long-term services and supports to clients that are functionally dependent and have a medical nursing need that require help with activities of daily living allowing them to receive services in their homes and community-based settings as opposed to a nursing facility.
Qualifying for Home and Community-Based Services (HCBS) typically depends on several factors including your health condition, level of care needed, income, and state-specific eligibility rules.
Medical Eligibility
- You usually need to have a chronic illness, disability, or condition that requires a level of care similar to what you would receive in a nursing home or hospital.
- A medical or functional assessment will be done to determine your needs. This can include evaluating your ability to perform activities of daily living (ADLs) such as bathing, dressing, eating, and mobility.
Meet Financial Eligibility
- HCBS is often Medicaid-funded, so you must meet income and asset limits set by your state’s Medicaid program.
- Some programs have more flexible financial limits or allow “spend down” of income/assets.
- You may need to provide documentation of income, assets, and expenses.
Assessment and Care Planning
- After application is approved, a care coordinator or nurse will assess your needs.
- A care plan will be developed that outlines the services you qualify for, such as personal care assistance, home health aides, meal delivery, transportation, or respite care.
Meet the Nursing Facility level of care criteria (i.e., they are functionally dependent and have a medical nursing needs); or Individuals who are dependent upon technological support and require substantial, ongoing skilled nursing care; and the health, safety, welfare of the individual must be safely maintained in the home when the nurse or personal care aide is not present; and Are determined to be at imminent risk of Nursing Facility placement; and • Are determined that community based care services under the waiver are the critical services that enable the individual to remain at home rather than being placed in a Nursing Facility.
